ZQuiet Pro Plus

The ZQuiet Pro Plus is a patient-friendly, dorsal style appliance that has evolved into one of the most popular therapy choices in dental sleep medicine. The two-piece construction provides lateral jaw movement enabling the patient to open and close normally. Sturdy fins on each side intersect with adjustable stops to maintain the lower jaw in a specific anterior position that optimally opens the airway and provides unobstructed breathing.

Designed to Maximize Patient Comfort and Compliance: The low profile design of the device is minimally invasive allowing for maximum intraoral space. There are no bulky hinges, posts, or mechanical hardware to interfere with the mouth opening or impede reflexive travel of the tongue. This provides greater comfort resulting in higher patient compliance.

Designed to Maximize Treatment Results: The micro-adjustment screws built into the angled stops on the buccal of the maxillary appliance provide the capacity to advance the mandible in 0.10 mm increments. Placement of the screw does not compromise tongue posture or impinge on the oral cavity.

Patient Specific Vertical: The occlusal aspects on the mandibular orthotic are fashioned to increase the vertical inter-incisal dimension to further assist in dilating the upper airway. The clinician can specify the vertical dimension to optimize performance for each patient.

The appliance consists of 2 acrylic splints bilaterally connected via an adjustable telescoping Herbst mechanism that allows for both lateral and sagittal movement of the jaw. The ZQuiet Pro Flex is the lightest and thinnest lab fabricated dorsal style appliance available and is nearly invisible when worn. The advanced thermoplastic is over10x stronger than cold cured acrylic which is what allows the appliance to be built with minimal bulk. It does not absorb moisture like typical dental acrylics and is virtually stain and odor resistant. The material allows for subtle flexion, is not brittle or prone to breakage.

Fin & Buttress Maintains Anterior Position: The obtuse angle bias of the fin and buttress junction necessitates anterior movement when the jaw opens. The titrating screws located in the mandibular fin provide the capacity to individualize the anterior advancement of the mandible in .25mm increments. Placement of the screw in the fin does not compromise tongue posture or impinge on the oral compartment.

Lingual Ramps Support Forward Tongue Position: Bilateral ramps on the lingual surface provide support and activate involuntary reflexes that facilitate the upward and forward position of the tongue creating greater pharyngeal space.

Best Practices

Taking Impressions

Instructions for Impressions:
Upper and Lower impression are needed by the lab to fabricate the appliance. Take full extension impressions into maxillary vestibule and mandibular sublingual area. The extension should go to the height of contour of the gingiva on all sides of the teeth.

Bite Registration:
Bite registration is an important step when constructing any sleep appliance. In order for the laboratory to construct an appliance that positions the jaws in the proper relationship to one another it is necessary to record this position (in the patient’s mouth) with hard wax or elastomeric material.

The starting bite position for any ZQuiet® Professional appliance is the maximum comfortable protrusive jaw position, typically between 50-70% of maximum protrusion. Be sure that the skeletal midlines are aligned when at the protruded position. You may use an Andra Gauge to record the bite and vertical.

Ordering Specifications For Vertical Opening
The vertical opening ensures enough room for the tongue — the larger the tongue, the more vertical necessary. Every patient is different, but the tongue size and primary position (during sleep) should be noted. We recommend experienced dentists use the Andra Gauge or George Gauge to denote the vertical opening in the bite registration. Alternatively the following is suggested guideline that can be used to specify vertical opening:

Female Patient –

  • sleeps primarily on her side and has no scalloping of the tongue. Vertical Dimension increase: 5.5 mm.
  • sleeps primarily on back and/or has a scalloped tongue. Vertical Dimension increase 6.5 mm.

Male Patient –

  • sleeps primarily on side and no scalloping of the tongue. Vertical Dimension increase 6.5 mm.
  • scalloping tongue and/or sleeps predominately on back. Vertical Dimension increase 8.0 mm.

It is important to take great care to ensure that the patient can maintain unstrained lip seal. Poor lip seal can lead to mouth breathing and subsequent uncomfortable drying of the oral mucosa.
